Application for Reasonable Adjustments (ARA) Form for Examinations

This is the form to be completed by the school principal or teacher to request adaptations or changes to an examination for a candidate with a disability, medical condition or learning need.

Application for Reasonable Adjustments (ARA) Form for DDE Assessments

To be completed by the Head of ADC or tutor to request adaptations or changes to a written assessment for a candidate who has a disability, medical condition or learning need.

Example Consent Form for Application for Reasonable Adjustments

Available to schools/ADCs as an example form they can reference when creating their own form to obtain permission from the candidate or their parent/guardian to share the candidate’s personal data with the ISTD before applying for reasonable adjustments.

Special Consideration Request Form

This is a Special Consideration form that must be completed by the school principal or teacher to request special consideration for a candidate who has experienced adverse effects due to temporary illness/injury/bereavement/trauma during or just before the examination. The application must be sent no later than 5 days after the examination has taken place.
